2026/2027 SY Transition Coordinator - Long Term Substitute Location: High School Position Summary: Supports students with individualized education programs (IEPs) or special needs to bridge the gap between high school and adulthood, post-secondary education, employment, or independent living. The ideal candidate will successfully manage job coaches, transition planning, career readiness, community and agency connections, vocational education, and structured learning experiences. Assignment Dates: February 3 � June 17, 2027 Compensation: $66,000. /yr.
